OSD is required each year to have parents complete and submit a new form to determine eligibility for free and reduced lunch. Each family is required to fill-out a new form each year even if the family does not qualify for this service. These forms are given the highest confidentiality; only the director of the food service department and the business manager has access to this information. When OSD publishes information regarding the free/reduced lunch program, student names are kept completely confidential; only the total numbers and percentages of eligible students are used.
